Policies & Guidelines

*Please read each section carefully.

Financial Policies

• Summer program tuition must be paid at the time of registration, either in full or in two installments. The second installment will be due by Friday, June 5.

• Payments can be made with credit/debit, cash, check or Zelle.

• Checks must be made payable to Dimensions Dance Theatre of Miami and mailed to 9210 SW 158 Lane, Palmetto Bay, FL 33157.

• Zelle payments must be sent to dimensionsdancetheatermia@gmail.com.

• If you would like to pay with a credit/debit card, please request a payment link at ddtmiami@gmail.com after completing your registration. All credit & debit card payments will be assessed a transaction fee of 4%.

• Summer program tuition is non-refundable.

Tardiness & Absences

• Students are responsible for staying informed of and managing their class and rehearsal schedules. Students are not permitted to enter class more than 15 minutes late.

• Please notify our Company Manager of any absence as soon as possible, before the scheduled class time, either by phone (305-801-3624) or email (ddtmiami@gmail.com)

Dress Code

• Any solid-colored leotard, pink or flesh tone tights and matching ballet slippers (for female-presenting dancers).

• Any fitted, color t-shirt or tank top with black tights or biker shorts (for male-presenting dancers).

• Hair secured neatly away from face; no loose hair.

• No jewelry (except stud earrings) is allowed for classes or rehearsals.

• For Contemporary classes: Solid-colored leotard or top, black jazz pants/yoga or athletic pants with solid-colored socks or ballet slippers.

Release and Waiver of Liability and Indemnity

I agree that the student’s use of Dimensions Dance Theatre of Miami (DDTM) & Miami Youth Ballet's (MYB) facilities, equipment, services or premises may involve risk of injury to persons, including those injuries described below. Student and Parent assume full responsibility. Parent and Student hereby release DDTM & MYB, it’s directors, employees and agents harmless from all liability for any loss or damage, for injury to student, whether caused by the active or passive negligence of DDTM or MYB. Student agrees to indemnify DDTM or MYB for any loss, liability, damage it may incur due to the presence of the Student in, upon or about the DDTM & MYB premises, facilities, services or equipment. Parent represents that (a.) the Student is in good physical condition and has no disability, illness, or other condition that could prevent Student from exercising without injury or impairment of health, and (b.) that Student has consulted a physician concerning an exercise program that will not risk injury to student or impairment of Student’s health. The risk of injury may include injuries arising in participation by Student in supervised or unsupervised activities at the school; injuries and medical disorders arising from exercising at the School such as sprains, broken bones, torn ligaments, torn muscles, heart attacks and strokes and accidental injuries occuring anywhere at the school. Parent agrees that the foregoing release, waiver and indemnity agreement is intended to be as broad as is permitted by the laws of the State of Florida and that if any portion is held invalid, it is agreed that the balance shall continue in full force and effect.

Media Release

I hereby grant permission to Dimensions Dance Theatre of Miami (DDTM) to use photographs and/or video of myself or my child for promotional and marketing purposes, including print materials, the DDTM website, and social media channels. I understand that these materials may be used without further notice or compensation, and I waive any right to confidentiality or approval of their use. I certify that I am the parent or legal guardian of the child, where applicable.
